All new clients will want to read my HIPAA, policy statement and No diagnosis provided documents to get oriented to my practice. The documents are in PDF file format.  

• If you are coming for bodywork, please complete the Brief Massage INTAKE form and bring to your first session.  

• If you are seeking interactive hypnosis, please complete the clinical Hypnotherapy INTAKE form and bring to your first session.  

• If you have experienced a motor vehicle injury, please also read the Motor Vehicle Accidents document, complete the patient’s insurance info and the
  medical massage INTAKE form and have your attending chiropractor or doctor complete the physician’s prescription ICD 10 codes form which is also required for treatment.
